Septic Tank Forms 54200: various sizes; 1000 gallon (mid-seam), 2000 gallon (mid-seam), 1250 gallon double compartment (mid-seam) or can be shortened for 1000 gallon double compartment form (mid-seam). 1500 gallon double compartment with slab roof. 1000 gallon short special form (approx. 5' x 10' x 4' tall) with separate slab roof. All forms operate manually with lever at bottom that pulls in the center seamed walls. Forms have been out of production and will require some maintenance to operate. Prices range from $1000 to $3,500
